Phyllis' obituary was published in "The Tribune Star" (Terre Haute, Indiana) on Wednesday, August 23, 2006:²

Phyllis J. "Phys" Smith BLOOMINGDALE -- Phyllis J. "Phys" Smith, 62, of Bloomingdale, died at 4:14 p.m. Tuesday, Aug. 22, 2006, in the Heart Center of Indiana in Indianapolis.   She was a beautician for 42 years and was the owner/operator of New Personality Beauty Salon on the west side of the square in Rockville.  She was born Feb. 1, 1944, in Clinton to Harold "Francis" Hinshaw and Pearl Cory Hinshaw.   Survivors include her husband, Donald L. Smith, whom she married Aug. 19, 1967, in Coloma; two daughters, Donna J. Smith and her husband, Jamie of Bedford and Katie P. Janney and her husband, Jay of Eaton, Ohio; four grandchildren, Jack Smith Wildman, Chris, Claire and Carson Janney; one sister, Delores Elmore and Patti of Marshall, Ind.; two brothers, Harold Hinshaw and his wife, LaVada of Bicknell and Charles Hinshaw and his wife, Gwen of Bloomingdale; and several nieces and nephews.  She also was preceded in death by her parents.   She was a 1962 graduate of Turkey Run High School and attended beauty school in Terre Haute.  She was a member of Coloma Friends Church and was generously active in her community.   Services are 10 a.m. Saturday in Barnes Mortuary, with the Rev. William Flanigan officiating.  Burial is in Coloma Friends Cemetery.  Visitation is 4 to 8 p.m. Friday, in the funeral home.
