He declared he was born Feby 17 1873, and was then residing in Camden, Arkansas where he was a self-employed farmer. He declared his nearest relative was his wife, Susie Henshaw of Camden. He was recorded as medium height, medium build, brown eyes, "mix grey" hair, with a notation where physical disabilities were normally recorded: "seems sound".⁴
