Ardmore, Okla.   DAVID L. HENSHAW David L. Henshaw, 46, of Ardmore, Okla., died May 17, 2005.   He was born Dec. 16, 1958, in Yale, Okla., to Ernest Leon and Joyce Stevenson Henshaw.  He worked as a painter for over 30 years and attended Provence Assembly of God Church.   He was preceded in death by a daughter, Kimberly Kae Henshaw.   Survivors include his wife, Cheryl Meine of the home; mother, Joyce Willis of Ardmore; son, Jeremy Henshaw of Springdale; stepson, Calvin Jeffcoats of Madill, stepdaughter, Michelle Crouch of Ardmore; sister, Tammy Collins of Wilson, Okla.; brothers, Ricky Henshaw of Ardmore and Paul and John Henshaw, both of Arkansas; three grandchildren; and grandmother, Elsie Henshaw of Springdale.   Memorial services will be at 11 a.m. Friday at Provence Assembly of God Church with the Rev.
