He declared he was born July 14 1895 in Troy, Kansas, and was then residing at 118 3rd St., McMinnville; self-employed at 202 Davis St., McMinnville. He declared his relative/contact was Mrs. Verna Bullis. He was recorded as 5'5" tall, 176 lbs., hazel eyes, brown hair, ruddy complexion, and "scar on nose" and "tattoo".12
