He declared he was born Nov 15 1887 in Berkeley County, West Virginia, and was then residing at 406 So. Kentucky, Martinsburg, Berkeley County, West Virginia, employed by the Housing Authority of The City of Martinsburg, West Virginia. He declared his relative/contact was Miss Louise Henshaw of the same address. He was recorded as 5'9" tall, 180 lbs., gray eyes, gray hair, ruddy complexion.7
