On Jan 20 1912 Bertha D. Henshaw was issued a U.S. passport from the U.S. Consul at Nanking, China. She declared she was born February 1st 1874 in Back Bay, Boston, Massachusetts, her permanent residence then being New York City. She declared she left the United States on November 27 or 28 1906 and was then "temporarily sojourning" at Wanchih, Anl[..unclear..] Riv. [unclear]. She declared she requested the passport for the purpose of "missionary work in Anchui, Kiangsi, Hupeh & Kiangsu" [all unclear]. She was recorded as age 37, 5' 3 3/8" tall, dark brown eyes, dark brown hair, fair complexion, oval face, round chin, "rather high" forehead, "rather large" mouth, straight nose.⁴

Bertha and family were shown in the 1927 city directory of Brookline, Massachusetts:⁵
