He declared he was born Nov. 6 1889 [note this is inconsistent with his declared age 18 and was presumably a mistake and should have been written Nov. 6 1898]. He declared he was then residing in Star City, Pulaski County, Indiana where he was a farmer employed by Elmer Hinshaw. He declared his nearest relative was Elmer Hinshaw of Star City. Although the name on the form was written "James Carl Hinshaw", he signed as "Carl Hinshaw". He was recorded as medium height, slender, blue eyes, brown hair.9 On the same day his father Elmer Hinshaw also registered for the draft.9 The draft board official who signed the draft registration was Carl's uncle, J.F. [James Franklin] Marker.9
