He declared he was born Jan 20th 1877, and was then residing at 310 Park Ave. in Burlington, a self-employed brick mason. He declared his nearest relative was his wife, Mrs. Charlie A. Hinshaw of the same address. He was recorded as short, medium build, blue eyes, dark hair.¹¹
